Mobile units that are open for more than 15 days in a calendar year require a full-time or part-time permit (if open less than 6 months per year). Each mobile unit or caterer is required to have a commissary and a commissary agreement with an approved kitchen. The commissary will be required to hold a separate food establishment permit. If the commissary is owned by a separate entity, a commissary agreement must be supplied to the Health Department. If a commissary ceases operations or the commissary agreement is canceled, the mobile unit or caterer must also cease operations and inform the Health Department of the change in status before finding a new commissary to operate from.
Fees for each Mobile Unit or Caterer Food Establishment Permit can be found as a full-time or part-time permit, price dependent on numbers of employees, are payable online at AllPaid (green button below). Once you have made the transaction at AllPaid, you must return to this site and fill out the application form on this page. We have no access to your credit card information and must have the application below and transaction number from AllPaid to match the application to your payment in order to provide a permit.
