Farmer's Market/Roadside Stand Vendor's Permit Application

Each Farmer’s Market/Roadside Stand Permit has a fee of $60, payable online at AllPaid (green button below). Once you have made the transaction at AllPaid, you must return to this site and fill out the application form on this page. We have no access to your credit card information and must have the application below and transaction number from AllPaid to match the application to your payment in order to provide a permit.

This permit is exclusively for vendors required to have a Food Establishment Permit at Farmer's Markets and Roadside Stands. This permit also covers any required off-site storage (for non-chicken eggs or frozen meats). These off-site storage areas may be inspected at the request of the Morgan County Health Department.

Please Note: Your permit will not be issued unless this form is filled out correctly and in its entirety. This permit is ONLY to be used for Farmer's Market locations and roadside stands and is not applicable to any other venue, such as festivals or craft fairs.

When you have submitted this form, you will be provided with an email to send a copy of your Certified Food Handler Manager Certificate.
Notes:

If only selling chicken eggs, you do not need a Farmer's Market Permit; you must have an egg license from the Indiana State Egg Board. (http://www.ansc.purdue.edu/iseb/)

If selling produce (whole & uncut), you should register with the ISDH Produce Safety Division. (https://www.in.gov/isdh/25773.htm)
